Dual Plating of the Distal Femur: Indications and Surgical Techniques

Abstract: Dual-plating of the distal femur is required in some cases to achieve stable fixation. The indications of a medial plate in addition to the lateral plate are medial supracondylar bone loss, low trans-condylar bicondylar fractures, medial Hoffa fracture, peri-prosthetic distal femur fractures, non-union after failed fixation with single lateral plate, poor bone quality and comminuted distal femur fractures (AO type C3). We recommend orthogonal plate configuration with locked plates by a single incision or dual … Show more

“…Anatomically precontoured built reduces soft tissue problems and acts as internal external fixator. 8 In addition, a locking compression plate has got distinct advantages of unicortical fixation and least chance of plate back out as the screw gets locked to the plate. Further, minimal soft tissue injury occurs when closed reduction is done and MIPO technique is used.…”

“…Historically, from the 1960s, onward distal femur fractures were treated conservatively with fracture bracing and traction with success rates up to 67-90% [ 8 ]. Only with the development new surgical techniques and technologies the approach has shifted from conservative to surgical stabilization.…”

“…Dual plating of the distal femur is believed to have a lower failure rate than single lateral plating as it eliminates cantilever bending by stabilising both sides of the distal femur. They also provide greater hold in the distal metaphysis particularly in osteoporotic bone 9 10…”
