Dual Sensory Impairment and Cognitive Decline: The Results From the Shelter Study

Abstract: Cognitive function declines faster in nursing home residents with DSI only when residents were not socially engaged. Therefore, residents with DSI might cognitively benefit from interventions to improve involvement in social life at nursing homes.

“…Sensory loss is associated with an array of detrimental clinical and functional outcomes, depending on the sense involved . Evidence supporting such relationships has focused primarily on single‐sensory modalities or dual impairments (typically vision and hearing), and dual deficits have ben found to have synergistic detrimental effects …”

“…Nursing home residents with dual sensory impairment (i.e., of concurrent vision and hearing) have a greater cognitive decline over time compared to those without sensory impairment (Yamada et al 2016). The primary auditory cortex (A1) is dynamically regulated by the neurotransmitter ACh; the regulatory mechanism modulated by ACh has been shown to integrate sensory information processing in A1 neurons (Metherate 2011).…”

“…In particular, the risks of worsening functional and cognitive abilities, social isolation, 6,7 falls 8 and sensory impairment should be highlighted. 7 The services provided by LSIEs must be sensitive to the needs of the elderly in order to reduce the risks associated with institutionalization, thereby providing comfort, security, and quality of life and preserving independence. Thus, this study aims to characterize long-term institutions for the elderly regarding physical-structural and organizational aspects.…”

“…17 Consequently, there may be little interaction among residents, excessive idle time, or only their involvement in passive activities. 7 A study on the sensory and cognitive decline of 4156 elderly people living in nursing homes in eight European countries reported that factors such as hearing and visual impairment are directly associated with functional decline. This decline is higher in elderly people who present social isolation, resulting not only from a lack of interaction with other residents, but also from a lack of activities such as reading, listening to the radio, and watching television.…”
