Ductal and lobular carcinoma in situ arising within an enlarging biopsy proven fibroadenoma

Abstract: The diagnosis via core needle biopsy of concurrent ductal carcinoma in situ and lobular carcinoma in situ within an enlarging previously biopsied benign fibroadenoma in women in their 40s is rare. Several case reports have described the occurrence of malignant changes within fibroadenomas, usually as an incidental finding following excision, and few reports have documented the transition of a fibroadenoma to malignancy. The current case report emphasises the importance of re-biopsying enlarging fibroadenomas, … Show more

“…Fibroadenoma (FA) is one of the most common benign breast tumors with a peak age of onset between 15 and 35 years [ 1 ]. Breast cancer rarely arises from FA and is detected only in 0.02–0.125% of FA-related cases, usually in patients over 40 years of age [ 2 ]. Furthermore, clinical, sonographic and mammographic findings are similar to those of FA alone [ 3 ], thereby complicating cancer detection.…”

“…Cancer may arise from the surrounding breast tissue, in the fissures of the FA, or the carcinoma may be completely confined within the FA. The age at presentation of these patients is 42.5 to 46.9 years-old, approximately 20 years later than the maximum age at presentation of patients with pure FA[ 1 , 6 , 10 , 12 ].…”

“…FA is a biphasic tumor composed of stromal (connective) and epithelial tissue that grows by estrogen and progesterone stimulation and is commonly found in pregnancy and lactation. During menopause, FAs suffer atrophy[ 3 , 4 , 6 , 9 , 13 ].…”

“…These 2 cases of breast DCIS arising within a fibroadenoma also clearly suggest that fibroadenomas should not be ignored and warrant close follow-up. Based on the course of the follow-up and diagnosis in the above cases and referring to the previous literature, a significantly enlarged mammary nodule with irregular morphology or uneven internal echogenicity under ultrasound may indicate the need for biopsy or surgical intervention[ 10 ].…”
