Introduction : Les technologies numériques dentaires de conception, simulation et fabrication sont en constante évolution. Il est possible de simuler numériquement des mouvements dentaires, ainsi que des déplacements des deux arcades et de réaliser des gouttières thermoformées fabriquées à partir de cette simulation. Objectif : Les auteurs présentent le couplage de la numérisation 3D intra-buccale avec la radiographie 3D de basses doses d'irradiation qui permet de réaliser un setup chirurgical très précis, de préparer des gouttières de repositionnement et de fabriquer des plaques d' ostéosynthèse. Le protocole de prise en charge orthodontico-chirurgicale par aligneurs utilisé par le premier auteur sera décrit en détail. Matériels et méthodes : Les auteurs présentent, au travers des cas cliniques exposés, les progrès réalisés en chirurgie orthognathique grâce à la 3D et au système des gouttières thermoformées Invisalign ® . Résultats : En observant les trois cas cliniques présentés, on peut constater que la durée moyenne de traitement était bien inférieure à la durée moyenne des protocoles orthodontico-chirurgicaux traditionnels.

The computer-aided design/computer-aided manufacturing (CAD/CAM) virtual orthodontic system produces customized brackets, indirect bonding jigs, and archwires based on a three-dimensional virtual setup. In surgical cases, this system helps to visualize the final occlusion during diagnosis and to efficiently plan individualized presurgical orthodontic treatments. A 20-year-old female patient with a skeletal Class III malocclusion, maxillary protrusion, and lip protrusion was successfully treated with orthognathic surgery and orthodontic treatment with maxillary first premolar extractions. The CAD/CAM system was applied for efficient treatment, with a total active treatment time of 16 months. In this case report, the applicability of the CAD/CAM virtual orthodontic system for orthognathic surgery cases is demonstrated. Suggestions are also made to overcome the limitations and to maximize the advantages of this system during orthodontic treatment of patients undergoing orthognathic surgery.

Introduction: In the literature, no consensus has been reached about orthodontic treatment time. Similarly, the determining factors of the latter have not yet been completely elucidated. Objective: The aim of the present article was to deepen the discussion on the major factors influencing orthodontic treatment time, as well as to present some strategies that have proven effective in controlling and shortening it. Method: Based on evidences found in the literature, the method focussed in providing the basis for clinical decision-making. Conclusions: Treatment time varies according to the type of malocclusion and treatment options. Orthodontist’s influence, patient’s characteristics and compliance are all decisive in determining treatment time, while the effects provided by orthodontic appliances and methods used to speed tooth movement up seem little effective.
