Dynamic 3-Dimensional Echocardiographic Assessment of Mitral Annular Geometry in Patients With Functional Mitral Regurgitation

Abstract: Background Mitral valve (MV) annular dynamics have been well described in animal models of functional mitral regurgitation (FMR). Despite this little, if any, data exists regarding the dynamic MV annular geometry in humans with FMR. In the current study we hypothesized that three-dimensional (3D) echocardiography, in conjunction with commercially available software, could be used to quantify the dynamic changes in MV annular geometry associated with FMR. Methods Intraoperative 3D transesophageal echocardiogr… Show more

“…In recent years, there have been several studies on mitral valve structure. [5][6][7][8] However, most studies focused on one part of the mitral apparatus. Due to the complexity and delicacy of the mitral valve, assessing the apparatus quantitatively and comprehensively remains a considerable challenge for physicians.…”

“…MVR without preservation of the mitral apparatus and chordal structures is not recommended because preservation of the mitral apparatus has shown to preserve LV geometry and systolic function, and improve survival [73][74][75][76]. In FMR, operative mortality for MV surgery is approximately 3 %-4 % [77][78][79][80].…”

“…Whereas conventional surgical ‘valve analysis’ is performed when the patient is on cardiopulmonary bypass under non-physiological circumstances, echocardiography provides anatomical information with a physiological perspective 52. TEE provides high-resolution dynamic 2D and 3D en face surgical views of the mitral valve, aortic and tricuspid valves that provide surgically relevant qualitative and quantitative information 23…”
