Dynamic MR Findings of Ductal Carcinoma in Situ within a Fibroadenoma

Abstract: Sci, Vol. 10, No. 2, pp. 129-132, 2011 We report magnetic resonance (MR) imagingˆndings of ductal carcinoma in situ (DCIS) within aˆbroadenoma in a 42-year-old woman. Dynamic MR imaging revealed the mass to have 2 components with diŠerent kinetics. A nodular area within the mass showed faster initial enhancement followed by earlier washout and was histologically proven to be DCIS. Dynamic MR imaging re‰ected diŠerences in vascularity between theˆbroadenoma and DCIS, and parameter color maps generated from t… Show more

“…Imaging studies may suggest characteristics of carcinomas arising from fibroadenomas [1416]. In sonographic examination of fibroadenomas, irregular shape and contour, extensive hypo-echogenicity, shadowing, echogenic halo and distortion of surrounding tissue are considered to be hints of suspicious malignancy [15].…”

“…On mammography, irregular shape, ill-defined spiculated margins or pleomorphic/linear micro-calcifications should also be considered signs of malignancy [14]. With dynamic MRI, benign fibroadenoma and DCIS/IDC can be differentiated according to differences in vascularity [16]. Parameter colour maps can also demonstrate the extent of DCIS within a fibroadenoma [16].…”

“…Typically, on MRI, fibroadenoma appears as a round or oval-shaped mass with a smooth margin. Fibroadenoma can exhibit variable enhancement; however, enhancement usually persists until the delayed phase ( 4 ). Only few reports described DCE-bMRI findings of carcinoma within fibroadenoma.…”

“…1 The mammographic features of carcinoma originating within a fibroadenoma include round, oval or lobulated mass, margin irregularity, suspicious linear or pleomorphic calcifications. 12 Fibroadenomas are seen as a focal round or oval shaped mass with smooth margins at MRI 12 . In our case radiologic imaging was reported as irregular countoured, heterogeneous hypoechoic mass with fibrotic shrinkage around at USG.…”
