E-Cigarettes—a review of the evidence—harm versus harm reduction

Abstract: The World Health Organization estimates there are 1.1 billion cigarette smokers across the globe and that tobacco related deaths number 7 million per year. Electronic nicotine delivery systems (ENDS) are available to contribute options for smoking cessation and include e-cigarettes, e-hookahs, vape pens, mods, and vaping. The growing use of ENDS, or e-cigarettes, in the US and globally across populations is dramatic. Although users may think that e-cigarettes are less harmful than combustible tobacco products,… Show more

“…Similar to a recent study conducted amongst 792 university students in northern Thailand [ 10 ], 143 (18.1%) respondents were current e-cigarettes users; the two most stated reasons for using e-cigarettes included: (1) being less harmful compared with conventional cigarettes and (2) being used to quit conventional cigarettes. The recently established evidence about the potential harm of e-cigarettes was nicotine dependence, toxicants and carcinogens from vapour, burn, and lung injuries [ 24 ]. There had been incidents of e-cigarettes related injuries such as an e-cigarettes or vaping use-associated lung injury (EVALI) in which e-cigarettes users experienced symptoms such as coughing, chest pain, breathing difficulty, stomach ache, nausea, vomiting, and in the worst cases, death.…”

“…David O. Warner serve as a "gateway" facilitating a transition to smoking conventional cigarettes, 54 a pattern noted in recent observational studies. [55][56][57] Evidence suggests that despite protestations to the contrary, companies producing electronic cigarettes actively promote youth use through strategies such as flavors and the renormalization of nicotine use (i.e., vaping as glamorous) to generate lifelong users of their products.…”

“…82,83 There are no data in patients save two case reports of problems with flaps in vapers that do not provide a convincing link. 84–86 Thus, even if vapor may prove “safer” than cigarette smoke in some respects, it is not “safe.” 54…”

“…ENDS are often promoted as a healthier alternative to traditional tobacco products due to using water vapor created by heating ‘e-liquids’ instead of combusting tobacco leaves; thus, ENDS users theoretically avoid inhaling thousands of chemicals known to be present in smoked tobacco [ 4 – 6 ]. People who are concerned about tobacco associated health conditions may turn to using ENDS, instead of smoking cigarettes.…”
