Early and midterm patency of the proximal anastomoses of saphenous vein grafts made with a Symmetry Aortic Connector System

Abstract: Once surgical errors had been ruled out, the 1-year patency rate of proximal saphenous vein grafts anastomosed with the aortic connector system was favorable. Poor left ventricular function and low graft flow during the operation were risk factors for midterm graft occlusion. Selection of the target vessel and its runoff may also be an important determinant of long-term patency.

“…Other investigators confirmed these findings (105, 106) although a Japanese group showed better patency (107). In contrast to what was the case in our study, some of the Japanese patients received anti-thrombotic therapy in addition to aspirin.…”

“…Thirty-two studies evaluated graft patency according to TTFM values, summarized in Supplementary Material, Table S3 [7, 12, 14–17, 30, 38, 44–68]. Currently, only one small, randomized clinical trial assigned patients to undergo isolated CABG with or without TTFM and/or IFI [69].…”

“…No differences were found in the rate of graft occlusion on CAG (30.9% vs 28.9%, imaging vs control, respectively, P = 0.82) [69]. Several observational studies reported that abnormal TTFM parameters predicted graft failure at 6 months to 1 year [7, 54, 70]. One study reported no predictive correlation between TTFM parameters and angiographic graft stenosis at 3 and 12 months [65].…”

“…The device was used in almost 40,000 patients 1 and initially showed good results with short-term (5-12 months) patency rates ranging between 90 and 100%. [2][3][4][5][6] Nevertheless, the device was withdrawn from the market after some mid-term reports showed that the incidence of graft occlusion was as high as 50%. 1,[7][8][9][10][11][12][13] Long-term follow-up studies are still scarce.…”
