Early Bactericidal Activity of High-Dose Rifampin in Patients with Pulmonary Tuberculosis Evidenced by Positive Sputum Smears

Abstract: We studied the early bactericidal activity of twice the standard dose of rifampin in subjects with pulmonary tuberculosis evidenced by positive smears. The observed mean 2-day activity was almost double that reported at the standard dose. Further studies are warranted to establish whether higher rifampin doses might assist in shortening tuberculosis treatment.Rifampin (RMP) is a key drug in standard antituberculosis regimens. The sterilizing properties of RMP in conjunction with pyrazinamide lay the foundation… Show more

“…Rifamycins have durable sterilizing activity against Mycobacterium tuberculosis, and the currently recommended 10 mg/kg dosage of rifampin, the most commonly used rifamycin, is at the low end of the dose-response curve (2)(3)(4)(5)(6)(7)(8)(9). Historically, selection of this rifampin dose seems to have been influenced by cost in the setting of incomplete dose-finding studies (10).…”

“…Increasing the R dose improves its pharmacokinetic properties including peak concentration and area under the concentration-time curve (AUC) [Diacon et al 2007]. Doubling the quotidian R dose from 600 to 1200 mg doubles its EBA and a dose of 1200 mg is generally tolerable [Diacon et al 2007]. Increasing R dosing from 10 to 13 mg/kg increases the peak concentration by 49% and the AUC by 65% [Ruslami et al 2006].…”
