Early Experience with a New Concept of Angioplasty Nitinol-Constrained Balloon Catheter (Chocolate®) in Severely Claudicant Patients

Abstract: In this preliminary experience, our protocol seems to be safe and effective in treating SFA and PA lesions in claudicant patients with satisfactory early and 12-month results.

“…Compared to standard PTA series, where reported primary patency are 52% with similar clinically driven TVR rates, an important advantage of the Chocolate Balloon is the absence of flow limiting dissections compared to those reported with standard PTA (60%–70%) . Sirignano et al evaluated the Chocolate Balloon in 84 consecutive patients with Rutherford 3 claudication and femoropopliteal disease, in a single center study. After Chocolate Balloon angioplasty, drug‐coated balloon angioplasty was utilized.…”

“…A single-center prospective single-arm study evaluated the use of Chocolate PTA balloon, systematically followed by DCB, in 84 patients with femoropopliteal lesions ( 14 ). The mean stenosis length was 69.4 ± 30 mm, and CTO was present in 65.5% of the limbs, with a mean length of 186 ± 82 mm.…”

“…A combination of either treatment strategy has been recently become available which is a paclitaxel‐covered balloon and constrained by a nitinol structure that creates pillows and grooves in the balloon. It also provides fast deflation and uniform rewrap (chocolate drug‐eluting PTA balloon catheter (C‐DEB PTA) . In this case report, we describe—to the best of our knowledge—the first successful percutaneous transluminal angioplasty with a nitinol‐structured (chocolate‐like) drug‐coated (paclitaxel) balloon technology (C‐DEB PTA) in a patient with TA.…”
