Early lung cancer with lepidic pattern

Abstract: The validity and clinical importance of the novel concept of ADC precursor lesions and LPA have been confirmed by clinical, radiological, morphological, and molecular data. Thereby, it has evolved into a valuable tool to aid in clinical decision-making.

“…The mean age of the patients was 65.1± 7.5 years, 93.3% self-reported race as white, 55.2% were female, 90.5% were ever-smokers, and 47% had stage I lung cancer. According to their growth pattern, we classified tumors into two subtypes as previous studies 12-14 : (1) lepidic predominant adenocarcinomas (n=55), and (2) adenocarcinomas without a predominant lepidic growth (n=62), among these cases, 11 had a small proportion of a lepidic component. Lepidic growth pattern was defined as involving alveolar septa with a relative lack of acinar filling.…”

“…The progression of GGNs from AAH, AIS and MIA to IAC is a continuous and dynamic process accompanied by the increment of malignancy [10]. The intraoperative classification of early lung adenocarcinoma is of great significance in determining surgical strategy [11].…”

“…The histological subtyping of ADC has a high prognostic (Warth et al , 2012) and predictive relevance (Tsao et al , 2015), with a gradual downgrading from lepidic (good prognosis) over acinar and papillary (moderate) to solid and micropapillary (poor). Thus, the histological patterns can be related to different stages of tumour progression (Weichert and Warth, 2014). Taking this into consideration, the observed correlation between histological growth patterns and mtDNA levels might be a stepwise reduction of mtDNA-CN during ADC evolution.…”
