Early opacification of silicone intraocular lenses: Laboratory analyses of 6 explants

Abstract: Most IOLs are enclosed in semipermeable packages to allow sterilization by ethylene oxide gas. During cleaning or disinfection of storage rooms, aerosolized solutions may introduce chemicals through the package and onto the IOLs. This may cause surface changes in the IOL, promoting opacification by water ingress in the aqueous environment. Cleaning and disinfection procedures of IOL storage areas should be monitored carefully.

“…We reported significant opacification observed as early as a few hours after implantation because cleaning agents and fumigants used in storage areas apparently penetrated the semipermeable packages contaminating the silicone IOLs preoperatively, causing changes that lead to influx of water and therefore opacification of the IOL optic ( Figure 14). 59,60 Other studies have described similar cases but without a clear history of preoperative contamination of the IOLs by exogenous molecules. 61,62 Brown discoloration and central haze in silicone IOLs observed from 15 to 60 months after implantation were described in the early 1990s.…”

“…The B63d disks became opaque white‐yellow at the end of the surgery and fibrin deposit was observed after 1 d (Figure D). These effects may be due to various types of processes, intraoperative or early postoperative such as surgical techniques or adjuvants, chemical contamination induced injury such as povidone iodine, calcification, interactions with adsorbed proteins likely serum albumin, presence of sodium hyaluronate (added during surgery), and alteration of the balance between procoagulant and anticoagulant reactions due to coumarin . During cataract surgery, the protein concentration and cell level in the aqueous humor increase because of the blood–aqueous barrier (BAB) breakdown.…”
