Early Prediction of Hypoxic‐Ischemic Brain Injury by a New Panel of Biomarkers in a Population of Term Newborns

Abstract: This research paper is aimed at evaluating the predictive role of a default panel of oxidative stress (OS) biomarkers for the early identification of infants at high risk of HIE and their validation through the correlation with MRI findings. A multicenter prospective observational study was performed between March 2012 and April 2015 in two European tertiary NICUs. Eighty-four term infants at risk for HIE (pH < 7, BE < −13 mmol/L, and 5′ Apgar < 5) were enrolled. Three were excluded for chromosomal abnormaliti… Show more

“…Negro et al . () found that infants who were later confirmed to have severe HI injury on MRI had increased oxidative stress levels 4–6 h after injury. MRI remains the gold standard for detection of brain injury after HI insult, but this is not normally conducted until days after the initial injury.…”

“…Males are more vulnerable to HI injury (Johnston & Hagberg, 2007). The increased vulnerability of males may be due in part to dysregulation of oxidative stress pathways, where males have decreased oxidative stress at birth, but increased upregulation of oxidative stress in response to HI (Negro et al 2018). Further, sex dimorphism has been seen in the efficacy of treatments.…”

“…Asphyxia is a relatively common source of neonatal brain damage (1), affecting ~2 in every 1,000 births (2). The hypoxic ischemia resulting from this oxygen deprivation can produce long-term sequelae, including intellectual disability and neurologic deficits, and nearly a quarter of neonatal deaths worldwide are attributable to birth asphyxia (2).…”

“…A burst of O2true_ during reoxygenation has been suggested to cause direct cellular damage to macromolecules and initiate cellular signaling pathways involved in cell survival and death, resulting in apoptosis in ischemic lesions. The neonatal brain is believed to be especially vulnerable to free radical damage, due in part to its low amount of antioxidants (1).…”
