Earwax of patients with hidradenitis suppurativa: A retrospective study

Abstract: BackgroundIn Western nations, hidradenitis suppurativa (HS) typically affects the apocrine gland-bearing skin of people of African origin, women, smokers, and individuals with obesity. The clinical characteristics of HS in Korea and Japan, however, are reportedly different from those in the West. We therefore hypothesized that wet earwax is associated with HS because most East Asian people are genetically predisposed to produce dry earwax.MethodsThe medical charts of 53 Japanese patients with HS were reviewed … Show more

“… 13 The reported positive effects of antiandrogen therapy may support the possibility that androgens play a negative role in HS.” 12 However, a new study about the role of gender in HS in a Korean population found the female-to-male ratio was 1:1.6. They described “a remarkable predisposition for HS among males” and noted that this finding was contrary to the findings usually reported in European studies, 14 but was in line with results shown in recent Japanese 15 and Turkish studies. 16 Two hypotheses based on genetic and environmental factors were developed to explain “Asian” vs “European” difference.…”

“…Regarding the association between the ABCC11 genotypes and apocrine glands, well‐developed apocrine glands have been observed in the EACs of patients carrying the wet earwax genotypes as opposed to the dry earwax genotype 7 , 8 . Furthermore, when compared with the general Japanese population, the proportion of patients with wet earwax is higher in those with hidradenitis suppurativa, which is an inflammatory skin disease in the apocrine gland–bearing skin 22 , 23 . It has been proposed that otitis externa with high epithelial turnover can contribute to acquired middle ear cholesteatoma 2 , 3 ; therefore, the maintenance of apocrine glands may be associated with this disease.…”
