Economic advantage of ‘self-made’ antibiotic-loaded spacer compared to prefabricated antibiotic-loaded spacer and spacer molds in two-staged revision arthroplasty

Abstract: Infection after total hip or total knee arthroplasty is a serious complication implying great costs for the health care system. Amongst the different treatment options, the two-step exchange using a spacer in the interval is a valid option. We evaluate the economic impact of our self-made antibiotic-loaded hip and knee cement spacers compared with prefabricated spacers and spacer molds. Costs to prepare self-made cement spacers are detailed for each spacer type. We also assess the intraoperative time spent fo… Show more

“…Although the success rates of these constructs were similar, the cost of the two types of articular spacer is substantially different. [ 3 ] The utilisation rates of primary THA are rising, as well as the cost of this already expensive procedure. The rise in the number and cost of THA revisions has had a substantial economic impact on healthcare.…”

“…Our self-made spacers are modeled using antibiotic-loaded bone cement and a humeral nail. Moerenhout et al[ 3 ] calculated that self-made hip spacers are at least 40 to 50% cheaper than prefabricated spacers.…”

“…Handmade articulating spacers are less stable, and it is difficult to produce a well-shaped and congruent articular surface (36). Hand-made hip and knee spacers can be 40 to 50% cheaper than prefabricated spacers and spacer molds (37). In fact, the mean price for self-made knee spacer is 514 CHF (450 EUR/505 USD) for non-articulated and 535 CHF (470 EUR/525 USD) for articulated ones.…”

For prefabricated knee spacers and knee molds spacers, the minimum cost is of 1050 CHF (922 EUR/1,030 USD) (37). This is a major economic advantage given the high costs of this surgical procedure (38).…”
