Economies of Scale and Efficiency Measurement in Switzerland’s Nursing Homes

Abstract: Summary This paper examines the cost efficiency in the nursing home industry, an issue of concern to Swiss policy makers because of the explosive growth of national expenditure on elderly care and the aging of the population. A stochastic cost frontier model has been applied to a panel of 1780 observations from 356 nursing homes operating over 1998–2002 in Switzerland. We compare the estimation results from different econometric techniques focusing on the specification of unobserved heterogeneity acr… Show more

“…According to Farsi et al (2008), Chattopadhyay and Ray (1996) and Worthington (2004), the literature on measuring efficiency in the nursing home industry dates back to the 1980s, and in the earlier econometric studies, the estimated relation reflected an average (based on best-fit) rather than an efficient or frontier cost function. Thus, inefficiencies were confounded with pure random shocks.…”

This frontier is usually estimated using a maximum likelihood method which assumes that any deviation from the technology (frontier) is composed of two parts, one representing randomness (or statistical noise) and the other inefficiency.…”
