Abstract:Two cases of ectopic pregnancy in the uterosacral ligament are presented. The cases are reported not only because of its rarity but also to arouse the thought of primary aetiology. With the increasing incidence of ectopic pregnancy due to assisted reproduction, gynaecologists are posed with the diagnostic challenge. The golden rule of management in extra-tubal pregnancy is to maintain high index of suspicion.
“…However, peritoneal pregnancy could cause sudden and serious bleeding even at earlier stages of pregnancy when a tubal pregnancy does not cause massive bleeding. Therefore, we reviewed the literature to evaluate the common symptoms of peritoneal pregnancy at 5 weeks of gestation . Consistent with previous reports, we observed no genital bleeding or gestational sac within the uterus in spite of massive intraperitoneal bleeding and abdominal pain (Table ).…”

“…Early diagnosis of peritoneal pregnancy is difficult but important because of the life-threatening nature of this complication. Several recent reports have demonstrated the effectiveness of laparoscopy in the diagnosis and treatment of peritoneal pregnancy [5,[7][8][9][10][11][12][13][14][15].…”
Key Clinical MessagePeritoneal pregnancy may cause severe abdominal bleeding without genital bleeding as early as the fifth week of gestation. Awareness that pregnancy can exist in unusual locations is imperative.

“…Peritoneal pregnancy involving the uterosacral ligament has been reported [4,5]. A negative human chorionic gonadotropin test had excluded this diagnosis in our patient.…”
“…The timely and appropriate use of transvaginal ultrasound and serum Beta hCG can facilitate early diagnosis and successful management by minimally invasive surgery. Had there been no evidence of intraperitoneal bleeding, we might have considered medical management and the diagnosis of a primary peritoneal pregnancy would have been missed altogether, a favourable site for implantation [8]. Poole et al, reviewed 225 cases of early abdominal pregnancies (which had completed less than 20 weeks gestation) from 1965 to 2009.…”

“…The most frequent complaint is abdominal pain [9] and other symptoms relate to the site of placental attachment. The trophoblast erodes into arterioles which lie underneath the peritoneum and can lead to massive intraperitoneal haemorrhage [8]. which leads to speculation about the true incidence of this condition.…”
