Ectopic thymoma in the paratracheal region of the middle mediastinum: a rare case report and literature review

Abstract: BackgroundThymomas generally arise from the thymus in the anterior mediastinum. Ectopic thymomas arising in the middle mediastinum are rare. We present a case of a thymoma arising from the ectopic thymic tissue in the right paratracheal region.Case presentationThe patient was a 67-year-old male who underwent an enhanced-computed tomography examination as preoperative staging for colon cancer. A 20-mm nodule in the right paratracheal region was found incidentally. Fluorodeoxyglucose (FDG) accumulation was detec… Show more

“…Only 2 patients were symptomatic, and the histologic type B3 was seen only in 1 patient. 1 The maximum size reported thus far is 7.5 cm, while our patient’s tumor was about 13 cm in size. Its histologic type was similarly a rare form of the ectopic thymoma B3.…”

“…With regard to their unusual site, the most important point is to differentiate them from other mediastinal tumors. 1 - 3 Different diagnostic imaging modalities such as chest X-ray, echocardiography, computed tomography (CT), and magnetic resonance imaging can be helpful in differential diagnostic and prognostic purposes. 4 We herein present a very rare case of a giant ectopic thymoma, as an unusual intrapericardial tumor, in an old man with a complaint of dyspnea and cardiomegaly in his chest X-ray.…”

“…Thymomas often occur in the anterosuperior mediastinum [6]. On the other hand, ectopic thymomas are considered to arise from thymic tissue that has failed to migrate into the anterosuperior mediastinum [7], accounting for only 4% of all thymomas [7], 8]. Weissferdt et al [6] reported that the incidence of ectopic thymomas was highest in the cervical region (45%), followed by the lungs (20%) and pleura (13%); other reported sites include the thyroid gland (6%), pericardium (7%), and middle/posterior mediastinum (4%).…”

“…EBUS-TBNA provides minimally invasive approach for diagnosing mediastinal tumors including thymomas and other thymic tumors (36,(42)(43)(44)(45)(46). The diagnostic yield of this technique in mediastinal masses has been shown to be up to 93% (87% in malignant and 96% in benign masses) (42).…”

“…The diagnostic yield of this technique in mediastinal masses has been shown to be up to 93% (87% in malignant and 96% in benign masses) (42). Most of the articles on EBUS-TBNA in thymic tumors are case reports (44)(45)(46). There are only few studies (6,36) owing to the rarity and anterior mediastinal location of thymic tumors not usually accessible by this procedure.…”
