Effect and safety of C7 neurotomy at the intervertebral foramen in patients with chronic poststroke aphasia: a multicentre, randomised, controlled study protocol

Abstract: IntroductionAphasia affects many stroke survivors; therefore, effective treatments are urgently needed. Preliminary clinical findings have suggested an association between contralateral C7–C7 cross nerve transfer and recovery from chronic aphasia. Randomised controlled trials supporting the efficacy of C7 neurotomy (NC7) are lacking. This study will explore the efficacy of NC7 at the intervertebral foramen for improving chronic poststroke aphasia. Methods and analysisThis study protocol reports a multicentre, r… Show more

“…The motor system is involved not only in understanding semantic meanings of specific actions but in integrating contextual information. This argument is supported by studies on pragmatics (Feng et al, 2017; Feng et al, 2021) and by clinical observations (Li et al, 2023). For example, the functional connectivity between the medial premotor cortex and right temporo-parietal junction (TPJ), a brain region subserving the inference of others’ mental states, increased with context-dependent indirectness of replies (e.g., a sentence “Nowadays, people are really beginning to enjoy opera.” would be an indirect reply to “Do you think that the audience liked my opera performance?”), demonstrating a general role of the motor system in pragmatic inferencing (Feng et al, 2017).…”

“…The mismatch between functional MRI scanning and behavioural evaluations was also a limitation that weakened the integrity of proposed theory on mechanism. Further explorations, such as randomized controlled clinical trial (Li et al, 2023) with higher levels of clinical evidence, will be carried out to explore more detailed mechanisms from comprehensive dimensions, including clinical evaluations as well as neurophysiological techniques.…”
