Effect of a Pragmatic, Cluster-randomized Controlled Trial on Patient Experience With Care

Abstract: Background-Health reform programs like the Patient-Centered Medical Home (PCMH) are intended to improve the Triple Aim. Previous studies on PCMHs have shown mixed effects, but High Value Elements (HVEs) are expected to improve the Triple Aim.
