“…As mentioned earlier, when patients are informed of the various modalities, 50% of them will choose a home based therapy. The proportion is only slightly lower (35%) in the case of unplanned urgent start (Rioux et al, 2011). Pre-dialysis education, if performed in an unbiased way, should lead to a large increase in the use of home modalities.…”

“…We and others have used a nurse navigator who meets with the patient with an urgent start and the family after the patient is stabilized to discuss options, including transitioning to home-based dialysis or starting the process for renal transplant. Of 228 patients with acute-start hemodialysis, the institution of the nurse educator program led to 49 patients transitioning to PD and 22 patients transitioning to home hemodialysis, a dramatic change from before the start of the program, where essentially all patients with acute starts remained on in-center hemodialysis (31).…”
