Effect of an Individually Tailored and Home-Based Intervention in the Chronic Phase of Traumatic Brain Injury

Abstract: ImportanceTraumatic brain injury (TBI) can cause long-lasting and heterogeneous difficulties that require an individually tailored approach to rehabilitation. However, high-quality studies of treatment options in the chronic phase of TBI are lacking.ObjectiveTo evaluate the effect of a home-based, individualized, and goal-oriented rehabilitation intervention in the chronic phase of TBI.Design, Setting, and ParticipantsThis study was an intention-to-treat parallel-group assessor-blinded randomized clinical tria… Show more

“…Possibly the functional aspects outweighed the influence of symptoms in the present cohort of individuals with TBI. The goal-oriented intervention focused on the individual problem areas, reduced symptoms, and improved overall health-related quality of life [17]. However, symptom burden was not associated with participation level or trajectories in the present study.…”

“…A randomized controlled trial (RCT) evaluating the effect of an individualized goaloriented intervention in the chronic phase of TBI [17] did not find between-group effects in social participation, as measured by the Social Relations subscale from PART-O. However, the study showed significantly improved generic health-related quality of life as well as reduced self-reported TBI-and anxiety-related symptom levels in the intervention group compared with the control group [17]. The trial included 120 persons with TBI with verified intracranial lesions and persistent symptoms at least two years since injury (time of inclusion) and was partly undertaken during the COVID-19 pandemic.…”

“…Enhancing federal support of state brain injury programs (Corrigan et al, 2021) can also promote community participation and improve life satisfaction. Additionally, exploring home-based care for patients with difficulty accessing traditional health care settings (e.g., see Beales & Edes, 2009; Cooper et al, 2007; Ritchie & Leff, 2022) because of transportation, physical, or cognitive reasons could also be beneficial (Borgen et al, 2023). Telehealth interventions have been effective in reducing emergency health care utilization and improving the well-being of homebound individuals with chronic illness (Gellis et al, 2014; Kitsiou et al, 2015; Kotb et al, 2015).…”
