Effect of Antipsychotic Withdrawal on Behavior and Sleep/Wake Activity in Nursing Home Residents with Dementia: A Randomized, Placebo‐Controlled, Double‐Blinded Study The Bergen District Nursing Home Study

Abstract: Antipsychotic drug withdrawal affected activity and sleep efficiency over the short term. Increases in total activity and impaired sleep quality after drug discontinuation should be monitored, because the long-term effect of these changes is not known. The NPI-Q and actigraphy are feasible tools that disclose relevant changes occurring during antipsychotic withdrawal in NH patients with dementia. Their use in clinical practice should be substantiated by larger studies.

“…The 30 patients included from institutions in Bergen were also enrolled into a substudy involving actigraphic assessment during the discontinuation trial. The results of this assessment related to selected corresponding BPSD data also included here, have been published previously (Ruths et al, 2004). At the end of the blinded 4-week intervention, and 1 and 3 months after this trial, data were gathered from the nursing homes regarding participants' current antipsychotic use.…”

“…[23][24][25] Such interventions have been found to be well tolerated and to not result in increased behavioral disturbances. Although current regulations in the nursing home setting require discontinuation trials, there are no such similarly strong policy-level recommendations in the non-nursing home setting.…”

“…From these 47 studies, 29 were excluded because they were part of an educational program to reduce antipsychotic prescription, 7 were excluded because of the observational study design and 1 was excluded because 2 studies [18,25] were the same study with a different follow-up period and outcomes reporting. Therefore, 10 studies [18,19,21,22,23,24,26,35,36,37] were entered into the qualitative data synthesis. Among the studies in the qualitative data synthesis, 1 study [35] was excluded from the meta-analysis because the medication discontinuation combined antipsychotics and other psychotropic medications and did not report the results separately.…”

“…Among the studies in the qualitative data synthesis, 1 study [35] was excluded from the meta-analysis because the medication discontinuation combined antipsychotics and other psychotropic medications and did not report the results separately. Finally, 9 studies [18,19,21,22,23,24,26,36,37] were entered into a meta-analysis. Each of the 9 studies can only provide some, but not all, data for different outcome variables.…”
