Effect of Collection Tube Type and Preanalytical Handling on Myeloperoxidase Concentrations

Abstract: BACKGROUND: Myeloperoxidase (MPO) has shown potential as a marker for cardiovascular disease. Limited studies have been published with a variety of sample types, resulting in a wide range of MPO values. Little is known or understood about the impact of collection tube type and preanalytical handling of specimens for MPO determination.

“…was similar for citrate and EDTA and more pronounced for heparin. These results are in agreement with previous studies showing higher total MPO contents in heparin plasma and serum than in citrate and EDTA plasma [28,29]. The SIEFED method allows reliable quantification of MPO activity from 0.156 to 10 mU/mL, what corresponds to 0.78-50 ng/mL pure MPO.…”

“…Overall, our findings for a heterogeneous group of patients who presented to an inner-city hospital with symptoms of ischemia suggest that MPO is a powerful predictor of cardiovascular risk, as has previously been shown in moderate-to highrisk ACS patients (4 -6 ). This study used EDTA plasma samples, in which MPO concentrations have been shown to be stable (13,14 ). Earlier studies that found higher MPO concentrations used serum or heparinized plasma samples, and the assays may have been prone to preanalytical stability problems caused by inadequate samplecollection procedures that led to inaccurate biomarker results.…”

“…Several studies have shown that heparin increases the plasma MPO concentration when used in vivo or in vitro, whereas MPO concentrations appear to be stable in EDTAcontaining plasma (13,14 ).…”

“…For example, the plasma concentration of MPO increases with time if human whole blood is kept at room temperature prior to plasma preparation, collection, and storage [120]. In addition, several studies have reported heparin plasma to contain more MPO protein than EDTA plasma [120][121][122][123], possibly due to ex vivo 'release' of MPO from blood cells/matrix by heparin [124]. This interpretation is supported by the observation that intravenous administration of heparin mobilizes MPO from vascular compartments, yielding higher values compared to baseline plasma concentrations, especially in patients with CAD [125].…”
