Effect of Continuous Intravenous Calcium Loading on Fibroblast Growth Factor 23 in Normal and Uremic Rats

Abstract: Fibroblast growth factor 23 (FGF23) is associated with mortality in patients with CKD. However, the mechanisms underlying stimulation of FGF23 remain to be investigated. We examined whether hypercalcemia induced by continuous intravenous (CIV) calcium (Ca) infusion regulates FGF23 levels in normal rats (Normal) and 5/6 nephrectomized uremic rats (Nx). Microinfusion pumps were implanted in the Normal and Nx rats for CIV Ca infusion, and blood, urine, kidney, and tibia were collected. The results showed an incre… Show more

“…Dietary Ca 2+ stimulates FGF23 production , and a Ca 2+ deficit lowers FGF23 . Ca 2+ ‐dependent regulation of FGF23 seems to be independent of PTH/vitamin D and possibly be due to a direct effect on Fgf23 gene expression via calcium channels . Ca 2+ signalling is a major driver of FGF23 production: store‐operated Ca 2+ entry (SOCE) via calcium release‐activated calcium channel (CRAC) Orai1 enhances gene expression of Fgf23 .…”

“…Another potential explanation for the association between serum albumin-corrected calcium level and infection-related death may be calcium-induced increases in circulating FGF23 and calciprotein particles (CPPs). In previous experimental studies, calcium overload and ionized calcium were shown to up-regulate FGF23 synthesis and secretion in bone 26,27 . Notably, leukocyte recruitment, which is crucial in infections including sepsis, was prevented by FGF23 12 .…”

“… 20 , 21 , 22 , 23 , 24 , 25 , 26 Calcium metabolism is an important factor among the variety of potential factors that regulate FGF-23 synthesis and secretion. 27 However, serum FGF-23 levels did not change after dialysate calcium concentration conversion. Log serum FGF-23 level was not correlated with serum calcium levels, but it was associated with predialysis serum phosphate level and intradialytic phosphate elimination.…”
