Effect of dental material fluorescence on DIAGNOdent readings

Abstract: Stained dental materials might affect DIAGNOdent readings and consequently result in false-positive diagnoses of secondary caries. Dental fillings should be polished prior to DIAGNOdent measurement.

“…This recommendation is in line with earlier findings, that unpolished filling materials can lead to false positive LF signals, while the same materials are below dentine caries detection threshold after polishing. 33 Karlsson found a very weak correlation (Spearman r not exceeding 0.31) between LF readings (old LF device DIAGNOdent 2095) and lesion depth of primary root caries lesions with and without cavitation. 22 However, the teeth that were used in their study were not stored frozen but stored in thymolized saline, and a significant drop of fluorescence signals could be anticipated.…”

“…The increased fluorescence could have been due to resin tags on the enamel surface. Elevated DIAGNOdent readings have been shown 28,29 to occur adjacent to composite resins; micromechanical interlocking of the dental sealant and enamel rods projects 25 mm to 50 mm into the etched surface. 5,30 It is also possible that some demineralization of the sealed enamel surface actually occurred.…”

“…This finding is in accordance with previous studies which concluded that fluorescence level of the teeth decrease or remain stable during the bleaching process, and DIAGNOdent as a caries measurement device would bring unreliable values for recent bleached teeth. [36][37][38] However, an additional measurement of DD values, after bleaching and prior to pH cycling could better illustrate the precise effect of bleaching on DD values. Further studies are needed to clarify the clinical relevance of the findings of this invitro study.…”
