Effect of Dexamethasone Eyedrops on Blood Glucose Profile

Abstract: Postoperative dexamethasone eyedrops have a greater effect on the blood glucose profile of diabetic patients than on nondiabetic patients. Clinicians should be alerted to this risk and may initiate appropriate follow-up in this patient subgroup.

“…This is due to the fact that most of our recruited patients are approximately three to five years past their initial diagnosis, and of these over half have concurrent systemic diseases such as ankylosing spondylitis (Table 1), making them even more likely to have received steroid therapy at some point. Although topical application of DEX has supposedly low systemic uptake, concentration of blood glucose levels has been shown to be increased in diabetic patients receiving DEX5657. Therefore, if a patient has received systemic steroid therapy prior to collection of blood samples, we cannot rule out potential impact on our results.…”

“…Two were letters to the editor (Barequet 2004; Rocha 2009), one had no comparison group (Carenini 1993), one modeled existing trial data (Mullins 2016), two studies reported on a mix of surgeries for various eye conditions (Maheshwari 1995; Yasuda 2016), one was a dosage study (NCT00758199), and one used oral rather than topical corticosteroids (Sabiston 1987). We excluded one other study because the report was not intended to assess vision outcomes (Bahar 2007). …”

“…Of note, most patients requiring non‐steroidal anti‐inflammatory drugs for pain control show a high incidence of gastrointestinal and cardiovascular risk factors [140]. Steroidal anti‐inflammatory drugs increase blood glucose levels, for example dexamethasone [141]. Therefore, it may be inappropriate to use potent anti‐inflammatory drugs to prevent the development of diabetes.…”
