This systematic review aimed to investigate whether minimally invasive root canal preparation ensures higher fracture resistance compared to conventional root canal preparation in endodontically treated teeth (ETT). A comprehensive search strategy was conducted on the “PubMed, Web of Science, and Scopus” databases, alongside reference and hand searches, with language restrictions applied. Two independent reviews selected pertinent laboratory studies that explored the effect of minimally invasive root canal preparation on fracture resistance, in comparison to larger preparation counterparts. The quality of the studies was assessed, and the risk of bias was categorized as low, moderate, or high. The electronic search yielded a total of 1,767 articles. After applying eligibility criteria, 8 studies were included. Given the low methodological quality of these studies and the large variability of fracture resistance values, the impact of reduced apical size and/or taper on the fracture resistance of the ETT can be considered uncertain. This systematic review could not reveal sufficient evidence regarding the effect of minimally invasive preparation on increasing fracture resistance of ETT, primarily due to the inherent limitations of the studies and the moderate risk of bias.

Objectives The aim of this study was evaluated longitudinally evaluate, after a 6-months period, the clinical performance of an alkasit- based material (Cention N, Ivoclar) in molars with root canal treatment. Methods The project was registered in the Brazilian Registry of Clinical Trials – REBEC (RBR-97kx5jv) on 19/01/2023. This randomized clinical trial that included 33 patients with an endodontically treated mandibular molar requiring restoration. Patients were randomly assigned according to the restorative treatment: control: restoration with Bulk-fill resin (Ivoclar - n = 17) and restoration with alkasite-based material (Ivoclar - n = 16). A self-etching adhesive was applied. Three calibrated professionals used the United States Public Health Service (USPHS) criteria to evaluate retention, secondary caries, marginal adaptation, restoration color, marginal pigmentation, and anatomical shape at seven days (baseline) and six months. The radiographic analysis assessed the presence of a radiolucent line adjacent to the restoration, the lack or excess of the material, contact point, and caries recurrence in both periods. Data were analyzed by Friedman, Kruskall-Walis, and Wilcoxon tests (α = 0.05). Results After 6 months of follow up, the Alksite-based demonstrated greater wear of the anatomical shape (p = 0.022), and a greater color difference compared to natural tooth (p = 0.0332), both with a statistically significant difference. All other criteria did not show significant differences between the materials or the analysis periods. Conclusion The use of Alkasite-based material on endodontically treated teeth influenced the color and anatomical shape performance, however did not influence the longevity and success of the composite restorations after six months. Clinical Relevance Alkasite Based material may be an alternative for the restoration of endodontically treated teeth

Background Inter-appointment medication of the root canals with appropriate intracanal medicaments has been advocated to improve root canal disinfection. Graphene oxide (GO) has shown promising antimicrobial activity against a wide range of microorganisms, besides the capability of carrying antibiotics. The current study aimed to compare the antibacterial activity of double antibiotic paste (DAP) and GO per se and in combination (GO-DAP) against Enterococcus faecalis (E. faecalis). Methods A total of 108 extracted human mandibular premolars were contaminated with three-week-old E. faecalis and subjected to a primary microbial assessment. The samples were categorized into 15 groups concerning the intracanal medicament (DAP, GO, GO-DAP, and control) and contact time (1, 7, and 14 days). Then, the root canals were medicated, incubated, and resubjected to a secondary antimicrobial evaluation. The colony-forming units (CFU) were counted to calculate the antimicrobial efficacy. The data were analyzed via the Kruskal–Wallis test (α = 0.05). Results GO-DAP was the only medicament that completely eradicated E. faecalis in 1 day. The percentage reduction of CFU/ml in the GO-DAP and DAP groups was higher than that in the GO group at all allocated contact times. Furthermore, a significant decrease of the CFU/ml was seen in the GO and DAP groups after 7 and 14 days of being medicated (P < 0.05). Conclusion Since GO-DAP improved root canal disinfection, this novel material can be introduced as a promising intracanal medicament against E. faecalis even in the short run.
