Effect of gentamicin dosing interval on therapy of viridans streptococcal experimental endocarditis with gentamicin plus penicillin

Abstract: This study compares the effects of a total daily dose of gentamicin given once a day (q.d.) or three times a day (t.i.d.) in the therapy of experimental endocarditis in rabbits caused by penicillin-susceptible, penicillintolerant, or penicillin-resistant viridans streptococci. Four isolates were used in vivo: one penicillin susceptible (MIC < 0.03 g/ml), one penicillin tolerant (MBC/MIC, <0.03/>32 g/ml), and two penicillin resistant (MICs ‫؍‬ 0.5 and 2 g/ml). Animals were infected with one of the four isolates… Show more

“…The advantage of once-daily administration of aminoglycosides for the treatment of endocarditis is presently under discussion (3,5,11,16,17,35; Dorscher et al, 30th ICAAC). The combinations of aminoglycosides administered once-a-day with penicillin or ceftriaxone have been found to be effective in studies using pencillin-susceptible (3,13,16,35), -tolerant (16), -resistant (16), and nutritionally variant (16) viridans streptococcal or staphylococcal (Gavaldà et al, 35th ICAAC) endocarditis models.…”

Based on these results, a successful clinical trial was conducted using the combination of ceftriaxone plus netilmicin, both administered once-a-day during 2 weeks to treat patients with streptococcal endocarditis (14).…”

“…Pharmacodynamic data obtained from animal models (17,18,37) have shown concentration-dependent killing by the aminoglycosides, which supports the idea that administration of these drugs once a day might be more effective. Along this line, gentamicin given once a day has been shown to be effective against experimental endocarditis due to viridans group streptococci (2,11,28) and experimental endocarditis due to Enterococcus faecalis (13,20) and in in vitro simulations of enterococcal endocarditis (14,32). To date, the efficacy of gentamicin administered once a day against MSSA has not been evaluated.…”
