Effect of hepatic arterial infusion chemotherapy of 5-fluorouracil and cisplatin for advanced hepatocellular carcinoma in the Nationwide Survey of Primary Liver Cancer in Japan

Abstract: Background:The efficacy of hepatic arterial infusion chemotherapy for the treatment of advanced hepatocellular carcinoma (HCC) remains unclear. Methods:The outcome of 476 patients with HCC who underwent hepatic arterial infusion chemotherapy with 5-fluorouracil and cisplatin (HAIC) were compared with 1466 patients who did not receive active therapy. Results:A survival benefit of the therapy after adjusting for known risk factors was observed (hazard ratio, 0.48; 95% CI, 0.41–0.56; P<0.0001). In propensity score-… Show more

“…Among the several agents available for HAIC, 5-FU plus CDDP has been the most frequently used in Japan, with a number of reports of favorable results (Table 3) (52)(53)(54)(55)(56)(57)(58), although insertion of an indwelling catheter is essential for this treatment. The reported response rates range from 8% to 86%, and the reported median OS range from 6 months to 33 months.…”

“…In addition to fluoropyrimidine (mostly 5-fluorouracil) monotherapy 29 , the combination of 5-fluorouracil and cisplatin has been the most commonly used HAI regimen in HCC (ref. 8,14,16,17,20 ). The efficacy of HAI of 5-fluorouracil/cisplatin combination seems to be higher than HAI of 5-fluorouracil alone 17 .…”

“…In a retrospective analysis comparing the outcomes of 476 patients treated with HAI with 1466 patients who did not receive any active therapy, survival was reported to be significantly prolonged in patients treated with HAI (ref. 14 ). Significant survival benefit was still evident when patients treated with HAI were matched with untreated patients using propensity score.…”

“…Besides surgical management, chemotherapy is another important therapeutic option for HCC, but it would cause severe toxicities which are commonly difficult to tolerate by HCC patients [3,4]; for instance, fluorouracil and doxorubicin, two drugs currently used for treating HCC, are not satisfactory since they can produce severe side effects and are not too effective [5]. Thus, searching for new candidate agents with better effectiveness and lower toxicity for treating HCC have received more and more attention.…”
