Effect of intra-aortic balloon pump support on microcirculation during high-risk percutaneous intervention

Abstract: Circulatory support with IABP increases microcirculatory flow in the smallest vessels of the sublingual mucosa. Our data support the hypothesis that intra-aortic balloon counter-pulsation increases coronary and microvascular perfusion, thus, improving microcirculation even in hemodynamically stable patients. The use of IABP may increase safety of complex PCI and decrease the risk of deleterious complications.

“…Using SDF imaging, den Uil et al documented the positive effect of intravenous low‐dose nitroglycerin infusion on tissue perfusion in acute heart failure. In addition, Jung et al observed improved perfusion of the sublingual microvasculature in patients with CS during intra‐aortic counterpulsation. Bedside evaluation of the sublingual microvasculature using SDF‐imaging is considered representative for microflow in other tissues, offering a potentially powerful tool for clinicians to target therapy …”

“…MFI of both small and large vessels decreased significantly immediately after a short period of IABP-support discontinuation and returned to baseline after restarting therapy. Again, no correlation with global hemodynamic parameters was found [17]. …”
