Effect of Methotrexate in the Treatment of Distal Interphalangeal Joint Extensor Tendon Enthesopathy in Patients with Nail Psoriasis

Abstract: To assess the effect of methotrexate on the development of distal interphalangeal joint extensor tendon enthesopathy in psoriasis, thirty-two people aged 34 to 57 years with nail psoriasis and distal interphalangeal joint extensor tendon enthesopathy (19 patients with Ps (psoriasis) and 13 with PsA (psoriatic arthritis) were started on methotrexate at 15 to 25 mg/week and the treatment was continued for 6 months). A total of 319 nails were examined. After six months of treatment, the thicknesses of the nail pl… Show more

“…The related report indicated that one of the pathogeneses of psoriasis is hypernomic activation of toll-like receptors in dendritic cells, which is basically believed to be crucial in initiating an immune response (Meng et al, 2017). To date, most studies have investigated feasible treatments for the disease, which may require the fundamental role for short-term improvement and long-term control, but there is no standardized therapeutic regimen for psoriasis and is incapable of resulting in the complete clearing of psoriasis (Al-Suwaidan and Feldman, 2000;Naldi et al, 2003;Krajewska-Wlodarczyk et al, 2018).…”

“…In a third study, patients with psoriasis ( n = 19) and psoriatic arthritis ( n = 13) were evaluated for morphologic changes in DIP extensor tendon enthesopathy on ultrasound before and after treatment with methotrexate. After 6 months of treatment, methotrexate resulted in a decrease in extensor tendon thickness in patients with psoriasis but not in those with psoriatic arthritis 33 …”

“…In the study by Savage et al., ustekinumab treatment resulted in the resolution of ultrasound‐diagnosed enthesitis in 62% of patients at 52 weeks 23 . Similarly, in a study by Krajewska‐Włodarczyk et al., 6 months of treatment with methotrexate resulted in a reduction in DIP extensor tendon thickness in psoriasis patients, but this effect was not seen in patients with psoriatic arthritis 33 . These findings suggest that there may be a brief window of time to proactively treat psoriasis patients with subclinical enthesitis, which may affect clinical outcomes and future disease progression.…”

“…A total of 753 articles were screened and 67 articles included, as depicted in the PRISMA flow diagram in Figure 1 9‐75 . The number of articles per technique varied substantially, ranging from 36 for US to just one on PET/CT.…”
