Aim To compare penetration depths of endodontic irrigants into the dentinal tubules of extracted teeth when using several activation methods. Methodology The root canals of 90 extracted human teeth were prepared to size 40, .06 taper. The straight and round‐shaped root canals were distributed randomly into six groups, and final irrigation was performed with EDTA and sodium hypochlorite as follows: (I) manual dynamic activation, (II) Ultrasonic, (III) Sonic, (IV) PIPS (photon‐induced photoacoustic streaming, (V) SWEEPS (shock‐wave enhanced emission photoacoustic streaming) and (0) control without final irrigation or activation. Subsequently, methylene blue was inserted into the canals and activated according to the groups (I–V). Teeth were sectioned horizontally, imaged under a light microscope, and dye penetration depths were measured in six sections per tooth and 24 points on a virtual clock‐face per section. Data were analysed statistically by nonparametric tests for whole teeth and separately for coronal, middle and apical thirds. Results Penetration of dye into the dentinal tubules was lowest for the controls. Median penetration depths amounted to 700–900 μm for groups I–V with differences in the apical thirds between group I and the other test groups. Minimum penetration depths were significantly greater for PIPS in the apical thirds (P ≤ 0.046). Conclusions Greater penetration depths occurred in the apical thirds for ultrasonic, sonic and laser‐induced activation compared to manual dynamic activation. PIPS was associated with deeper penetration of irrigants. The novel SWEEPS mode did not increase irrigant penetration.

The aim of this in vitro study was to compare the percentage of penetration of three final irrigant into the dentinal tubules after application of several irrigation techniques. Mandibular premolars were divided into nine groups (n:10): chlorhexidine (CHX) + conventional syringe irrigation (CI), CHX + EndoActivator (SI), CHX + Er,Cr:YSGG laser (LAI), QMix + CI, QMix + SI, QMix + LAI, Irritrol + CI, Irritrol + SI and Irritrol + LAI. Following irrigation, all roots were horizontally sectioned at 2 mm and 5 mm from the apex and examined using a confocal laser scanning microscopy. QMix exhibited a significantly higher penetration percentage than CHX at the apical section (P < 0.05). In the Irritrol group, SI displayed a significantly higher penetration area than CI in the middle section. Statistically significant differences were also determined between middle and apical section (P < 0.05). Use of QMix with different agitation technique seems advantageous in dentinal tubule penetration.

Key Clinical MessageSuccessful management of a rare case involving both dens evaginatus and dens invaginatus in the same tooth, monitored over a 24‐month follow‐up.AbstractDens invaginatus (DI) is a congenital dental anomaly characterized by the presence of a tooth that resembles a “tooth within a tooth.” Conversely, dens evaginatus (DE) is a developmental anomaly distinguished by an additional tubercle or cusp on the tooth's crown. Both DI and DE can result in pulpal and periapical diseases in the affected tooth. This article presents a case of a healthy 14‐year‐old male with recurrent swelling under the chin and a wound with pus drainage on the right side of the submental area, associated with his left mandibular lateral incisor affected by both DI and DE. Clinical and radiographic examinations revealed that the tooth was necrotic and had a chronic apical abscess. Cone beam computed tomography (CBCT) confirmed Oehlers' type III DI and a talon cusp on the lingual surface of the same tooth. The patient underwent orthograde endodontic treatment. Passive ultrasonic activation of a 5.25% NaOCl solution and a mixture of Ca(OH)2 and a 2% chlorhexidine solution were utilized to effectively clean and eliminate the persistent pus discharge. After the resolution of the patient's symptoms, the apical third of the root canal and the invaginated space were filled with a plug of mineral trioxide aggregate (MTA), while the remaining root canal was filled using a sealer‐based obturation technique. A 24‐month follow‐up visit revealed complete bone regeneration in the previously affected periradicular tissues.
