Effect of Prenatal Antiarrhythmic Treatment on Cardiac Function in a Twin Pregnancy

Abstract: We present a case of supraventricular tachycardia affecting one fetus in a twin pregnancy. Before and after treatment with flecainide and cardioversion, we examined conduction times and heart rate variability (HRV) in both twins on the basis of magnetocardiography. Cardiac conduction times increased in both fetuses but HRV showed opposing effects with a number of HRV measures. This case demonstrates that magnetocardiography not only enables identification of fetal arrhythmia, but also permits the investigation… Show more

“…In addition to these findings in adults, case reports have been published which describe reduced fetal HRV after prenatal treatment with Flecainide [36,37]. In line with these clinical observations, we found a reduction of shortterm variability at comparable concentrations of Flecainide.…”

“…If the rate is >220 bpm, use of sotalol or amiodarone may be effective 78,8587. Flecainide may also be effective, but should be used with caution in the presence of ventricular dysfunction, owing to an increased risk of adverse events (conduction disturbances, cardiac arrest, and death) associated with this agent 59,8890. A short (1–2 week) course of dexamethasone 4–8 mg daily has been beneficial in cases where viral or isoimmune myocarditis is suspected 81,82…”

“…Clinical studies that examined the effect of Flecainide in patients after acute myocardial infarction and in patients without structural heart disease describe a decrease of time domain parameters of HRV 11,12 . Furthermore, case reports describe reduced fetal HRV after prenatal treatment with Flecainide 13,14 .…”
