Effect of the introduction of an emergency general surgery service on outcomes from appendicectomy

Abstract: Background: Appendicectomy is a common general surgical emergency procedure and may be used as a surrogate marker to evaluate quality in surgical management. The aim of this study was to assess the outcomes of appendicectomy before and after the introduction of a consultant-led emergency general surgery (EGS) service at a large metropolitan tertiary referral centre. Methods: A retrospective historical control study was performed that included all adult patients undergoing appendicectomy during two 18-month peri… Show more

“…Patients admitted during the call period would be under the care of that consultant for the whole duration of the admission. Should the patient require surgery, there were often other competing priorities including on‐ or off‐site elective work . With the ASU model, the on‐call acute care surgeon would be free from elective work, which would mean that the surgeon is available should prompt assessment or surgery be required.…”

“…Many other centres followed suit, with generally positive results . However, only 11 sites have reported outcomes in appendicitis, only one study comprised >1000 patients and none were from South Australia . Management of emergency general surgical presentations at our metropolitan tertiary referral hospital changed from the Traditional to the ASU model on 1 August 2012.…”

“…As opposed to these, a substantial proportion of SBO cases are managed conservatively. Secondly, our patient cohort was substantially older (mean age over 60) compared to the studies, which analysed appendicectomy and cholecystectomy (mean age 30–40) …”

“…The impact of the ASU on the outcomes of appendicectomy and cholecystectomy has previously been assessed. Demonstrated benefits included reduced time to theatre and overall hospital stay, decreased after‐hours operation, use of preoperative computed tomography (CT) imaging and post‐operative complications . To our knowledge, only three small studies have analysed the impact of the ASU on the management and outcomes of SBO .…”
