Effect of Tranexamic Acid Administration on Remote Cerebral Ischemic Lesions in Acute Spontaneous Intracerebral Hemorrhage

Abstract: IMPORTANCE Hyperintense foci on diffusion-weighted imaging (DWI) that are spatially remote from the acute hematoma occur in 20% of people with acute spontaneous intracerebral hemorrhage (ICH). Tranexamic acid, a hemostatic agent that is under investigation for treating acute ICH, might increase DWI hyperintense lesions (DWIHLs).OBJECTIVE To establish whether tranexamic acid compared with placebo increased the prevalence or number of remote cerebral DWIHLs within 2 weeks of ICH onset. DESIGN, SETTING, AND PARTI… Show more

“…All raters were blinded to outcome measures and TICH-2 treatment allocation. We assessed available source images from the TICH-2 MRI substudy 19 for possible or probable CAA according to the modified Boston criteria. 3 Type of ICH For this analysis, we classified participants into 1 of 3 three groups according to hematoma location and presence of CT markers of CAA (FLP and SAH) or probable CAA according to modified Boston criteria on MRI: nonlobar ICH, lobar non-CAA ICH, and lobar CAA-ICH.…”

“…Since the patient had uraemia, hypertension and coronary disease, surgery was relatively contraindicated, and conservative treatment was considered more suitable instead. Tranexamic acid may be safely and effectively administrated for acute spontaneous intracerebral hemorrhage (14). On the other hand, hyponatremia (Na <135 mmol/l) may be easily overlooked in certain occasions, and it has been shown to correlate with worse outcome in various studies on patients with SBH (15).…”

“…In this context, the study by Pszczolkowski and colleagues provides novel findings. The authors performed a post hoc analysis of patients with ICH who were enrolled in TICH-2, an international, placebo-controlled, phase 3 randomized clinical trial of tranexamic acid that was administered within 8 hours of ICH .…”
