Effect of Ultrasound‐Guided Intercostal Nerve Block Versus Fluoroscopy‐Guided Epidural Nerve Block in Patients With Thoracic Herpes Zoster: A Comparative Study

Abstract: Objectives To compare the efficacy of a conventional fluoroscopy‐guided epidural nerve block and an ultrasound (US)‐guided intercostal nerve block in patients with thoracic herpes zoster (HZ). Methods This work was a comparative study of 38 patients with thoracic HZ pain and a chest wall herpetic eruption, aged 18 years or older, with pain intensity of 5 or greater on a numeric rating scale (NRS) for less than a 1‐month duration. Patients were consecutively enrolled and assigned to 2 groups in which the interv… Show more

“…9 Lidocaine makes the lesion area reach a painless state quickly and plays the role of muscle relaxant, vasodilator and improved microcirculation. 29 Dexamethasone provides antiinflammatory, analgesic and immunosuppressive effects and prolongs the effect time when combined with local anesthetics. [30][31][32][33] The application of nerve block has been proven to be safe in the treatment of herpes zoster.…”

“…As in other peripheral blocks, the use of steroids as adjuvants in the intercostal nerve block has been shown to prolong the block duration. Lee et al [32] compared the efficacy of conventional fluoroscopy-guided ESP block and USG-guided intercostal nerve block in patients with herpes zoster. Block was applied with 5 mL of 2.5 mg dexamethasone and 0.5% lidocaine combination.…”

“…Any procedure involving the chest wall (thoracic surgeries), as well as rib fractures, could be an indication for ICNB ( 16 ). Indications outside the operating room include postsurgical analgesia after thoracostomy and postherpetic neuralgia ( 17 ). Thoracic epidural analgesia (TEA) has been considered the gold standard for postoperative pain control after thoracic surgery and was an integral part of early Enhanced Recovery After Surgery (ERAS) protocols for many years.…”
