Effect of Viral Load and Drug Resistance on Mortality among Chinese HIV-Infected Patients Receiving Antiretroviral Treatment

Abstract: Background: The Chinese National Free Antiretroviral Treatment Program (NFATP) for HIV-infected patients has saved lives. However, there is no data on the effect of HIV drug resistance on mortality among Chinese patients receiving antiretroviral treatment. Methods:We linked patient records from two national databases: baseline HIV drug resistance and viral load data were obtained from four national drug resistance surveys in 2004, 2005, 2006 and 2009, and all-cause mortality was ascertained in a prospective wa… Show more

“…Furthermore, individuals with VL ≥ 10,000 copies/ml and without HIV-drug resistance were also 1.7 times as likely to die [36]. Although we did not test for HIV resistance in our cohorts with a high pretreatment viral load, HIV-drug resistance in ART-naïve individuals with high viral load was suggested as the reason for the increased mortality in the study of Wang et al [36].…”

“…Specifically, in the study by Wang et al among Chinese adults (16–80 years old), patients having VL ≥ 10,000 copies/ml and HIV-drug resistance were two times as likely to die compared to patients with VL < 1,000 copies/ml [36]. Furthermore, individuals with VL ≥ 10,000 copies/ml and without HIV-drug resistance were also 1.7 times as likely to die [36].…”

“…All patients who received treatment from 2005 onwards were chronologically contacted for recruiting to the study; detailed subject recruitment procedure has been previously described. [4] The eligibility criteria include: receiving HIV antiretroviral therapy through NFATP from 2005 to 2009, being 18 years or older, and willingness and consent to participate. Treatments in the NFATP were first-line ART regimens consisting of 2 NRTIs [azidothymidine (AZT)+didanosine (DDI) or stavudine (D4T)+lamivudine (3TC)] and one NNRTI [nevirapine (NVP) or efavirenz (EFV)].…”

“…The increasing use of ART would lead to an increase in the incidence and prevalence of drug resistance especially in developing countries under WHO guidelines [3]. Previous studies on the prevalence of HIV drug resistance (HIVDR) in China and their associated factors [4], [5] have served as assessments of the HIVDR consequences of China’s National Free Antiretroviral Treatment Program (NFATP). However, incidence of HIVDR is also a critical indicator of the future success of HAART but currently remains poorly studied in China.…”
