Effective treatment and prevention of attempted suicide, anxiety, and aggressiveness with fluoxetine, despite proven use of androgenic anabolic steroids

Abstract: The treatment of a man who attempted suicide after experiencing symptoms of anxiety and aggressiveness associated with the use of androgenic-anabolic steroids (AAS) is described. This report includes 30 days of inpatient treatment and a 6-month follow-up. Regular use of fluoxetine apparently prevented the onset of anxiety, depression, aggressiveness, and suicide ideation, even with the concurrent use of AAS. The urinary concentration of androgens, metabolites of AAS, and fluoxetine were monitored through analy… Show more

“…Although some controlled studies described a dose‐dependent association between AAS use and the onset of psychiatric symptoms, 13–15 this association is not always observed in ecological use of AAS 8 . In an attempt to reduce self‐report bias, recent studies have employed urinary analysis to detect and quantify the use of AAS in ecologic settings 16,17 . Since urinary analysis of androgens are not usually available in the clinical setting, seric sexual hormones affected by the use of AAS can also be used to investigate the likelihood of AAS use 18 .…”

“…The continuation or resumption of AAS use driven by symptoms of hypogonadism [ 97 100 ] was a significant finding, together with wider issues related to sexual health [ 101 ]. Harms associated with IPED use were commonly described [ 18 , 19 , 102 , 103 ], together with the risk environment [ 6 ] and efforts by individuals to mitigate these adverse consequences [ 104 ].…”

“…As part of service provision there were also calls for comprehensive testing including physiological indicators of harm [ 112 ] together with testing for BBVs [ 126 ] and psychosocial support regarding body image. Such testing for BBVs could draw upon the improved understanding of injecting beliefs and behaviours [ 101 ], including BBV risks [ 25 , 27 , 102 – 107 ], evident in the review. Information of nutrition and exercise [ 113 ], building self-esteem [ 97 ], psychological services to address dependence and muscle dysmorphia [ 82 , 97 ], and other body image vulnerabilities [ 59 ] were also considered necessary.…”

“…However, they often ignore the possible adverse effects and the severity of their actions, and even if they know about them, they are willing to take risks. These include possible behavioral changes, anxiety [ 6 ], increased aggression [ 7 ], hepatotoxicity [ 8 ], cardiotoxicity [ 9 ], abnormalities of the reproductive system [ 10 , 11 ], and many others [ 12 ]. Particularly alarming, however, is a tendency to engage in criminal activities [ 13 ], along with the growing number of case reports of fatal medical conditions connected to the misuse of AASs [ 14 , 15 , 16 , 17 , 18 ], potentially resulting in sudden deaths [ 19 , 20 , 21 , 22 ].…”
