Effectiveness of a transluminal endoscopic fundoplication for the treatment of pediatric gastroesophageal reflux disease

Abstract: The TIF procedure can complement the current surgically and medically available options for children with GERD, especially in complicated patients such as those with NI. However, complications including hemorrhage emphasize the potential risk of the procedure. Further studies with more patients and a longer follow-up course must be conducted to better assess efficacy.

“…Chen et al demonstrated the use of TIF in 11 pediatric patients with average age of 16.5 years and noted a resolution of GERD symptoms in 91% of patients and resolution of reflux on pH probe study on all patients who underwent follow-up testing. 36 However, long-term follow-up of this cohort published 5 years later noted recurrent symptoms in 55% of patients, reintervention in 36%, and resumption of PPI in 82% of patients. 37 Recently, several authors have published the results of TIF combined with hiatal hernia repair.…”

“…Therapeutic endoscopic procedures are rarely indicated and should only be performed in units where there is evidence of experience. The transoral incisionless fundoplication procedure can complement the current surgically and medically available options for children with GERD, especially in complicated patients such as those with neurological impairment [92]. Surgery is indicated when symptoms are life-threatening or when a child beyond the age of 2-3 years is depending on chronic treatment with antiacid medications.…”
