Effectiveness of an interactive telerehabilitation system with home-based exercise training in patients after total hip or knee replacement: study protocol for a multicenter, superiority, no-blinded randomized controlled trial

Abstract: BackgroundTotal hip or knee replacement is one of the most frequently performed surgical procedures. Physical rehabilitation following total hip or knee replacement is an essential part of the therapy to improve functional outcomes and quality of life. After discharge from inpatient rehabilitation, a subsequent postoperative exercise therapy is needed to maintain functional mobility. Telerehabilitation may be a potential innovative treatment approach. We aim to investigate the superiority of an interactive tel… Show more

“…It is difficult to compare the findings of this case series with those of other studies using telehealth due to the specific nature of HSS@Home: we treated only patients who were part of Medicare’s CJR bundled payment program, in an urban US setting, without the use of additional devices, and integrated into the hospital’s EMR. The studies we reviewed either did not specify whether patients were part of a bundled care arrangement or were conducted outside of the USA, where payment models differ [3, 8, 1214, 19, 23]. Payment model and region may inform discharge tendencies.…”

“…17 Furthermore, at the present time, two randomized controlled trials are being performed to assess telerehabilitation outcomes in patients who received THA. [18][19][20] Patients value personal satisfaction above all else. Often, they relate satisfaction to the quality of care they perceive to have received.…”

“…Two physical performance tests were conducted on the fifth day. The first was the five times sit to stand test (5xSTS), which is a valid, reliable measure of functional lower limb muscle strength, which has been used after hip surgery in elderly people 25. The 5xSTS entails measuring with a hand-held stopwatch the time required to rise from a standard height (0.43 m) chair with armrests, five times as fast as possible.…”
