Providing a continuum of care (CoC) is important strategy for improving maternal, newborn, and child health (MNCH). Japan's current very low maternal and infant mortality rates suggest that its CoC for MNCH is good. In this paper, we attempt to clarify how CoC and low mortality rates are being maintained in Japan, by examining the entire MNCH service provision system. First, we examine two important tools for integrated service provision, the Maternal and Child Health (MCH) Handbook and registration of pregnant women with local governments, both introduced in 1942. Second, we explore the incentives provided by the MNCH system that prompt actors to participate in it. The three actors identified are service users (e.g., mothers and babies), medical professionals, and local governments. Through system design, all three actors benefit in ways that incentivize them to use MNCH services, which consequently connects service users with resources: all service users regardless of financial status, nationality, and location can receive free MNCH services such as antenatal care, assistance with childbirth, postnatal care, and immunizations; using the handbook, service users obtain health information, and medical professionals obtain the health records of pregnant women and their children as well as access examination fees from the local government by submitting vouchers in the handbook; local governments can then identify pregnant women for follow-up and provide health information and administrative services. As a result, the coverage rate of the MCH Handbook has reached 100% and MNCH services coverage could potentially reach the same level.

Background. The Mother and Child Health Handbook (MCHH), a tool used by almost all parents in Japan, serves as a record book shared by parents and health providers to monitor maternal health care throughout the perinatal period, track the child’s health and growth, and provide educational information. Methods. A review of the existing literature was performed by narrative review using electronic databases with the search term “Maternal and Child Health Handbook” from January 1980 to February 2016. Results. Twenty-eight papers were obtained: 3 review articles, 17 original articles, 2 brief reports, 2 letters, 1 research note, and 3 proceedings. After the MCHH was initiated in 1947, Japan’s infant mortality rate decreased to 2.6 per 1000 live births in 2007, and it is still decreasing. Information recorded in the MCHH at antenatal examinations can be used to evaluate a child’s risk of obesity, cardiovascular disease, endocrine disease, mental illness, and infectious disease. Utah’s Department of Health implemented a program called “Baby Your Baby” in 1987 based on the Japanese MCHH; this included a similar booklet with family records and educational information. Thus, the MCHH is a unique tool in Japan that has influenced other countries to adopt similar programs. Conclusion. We will confirm the importance of the MCHH’s role in promoting health and open dialogue.

The period of 1000 days of life plays an important role in determining the quality of children's growth and development. Accessibility of quality health services through the MCH handbook is an important strategy for improving maternal and child health. The purpose of this review is to present an overview of the use and usefulness of the possession of a specific MCH handbook on maternal, infant and child outcomes, as well as the potential for further research development. The method of systematic study and study search through the ProQuest, EBSCOhost, and Scopus databases, with the keywords maternal and child health handbook, home based record, mother, newborn, infant, and child, published from January 2008 to October 2018. Furthermore, filtering of articles and data abstraction. A total of 56 articles on the use of the MCH handbook that met the eligibility/inclusion criteria were screened, and 16 articles were selected for a systematic review. The variety of existing studies, has proven the benefits, functions, and effectiveness of the MCH handbook on the health of mothers, infants, and children. The categories of use and use of MCH handbook are integrated with home-based records, namely as an effective tool to facilitate improvement in health-seeking behavior; continuous care; health communication, information and education; as well as home based health records. In order to fill the gap, further research can be directed at evaluating especially premature babies/LBW as part of a population of newborns with high risk. The use of digital technology in improving the implementation of interventions and the accessibility of MCH handbook is also important to be developed.
