Effectiveness of informational decision aids and a live donor financial assistance program on pursuit of live kidney transplants in African American hemodialysis patients

Abstract: BackgroundAfrican Americans have persistently poor access to living donor kidney transplants (LDKT). We conducted a small randomized trial to provide preliminary evidence of the effect of informational decision support and donor financial assistance interventions on African American hemodialysis patients’ pursuit of LDKT.MethodsStudy participants were randomly assigned to receive (1) Usual Care; (2) the Providing Resources to Enhance African American Patients’ Readiness to Make Decisions about Kidney Disease (… Show more

“…The Providing Resources to Enhance African American Patients’ Readiness to Make Decisions about Kidney Disease (PREPARED) is theoretically driven and was designed to address the risks and benefits of treatment options for kidney disease, including peritoneal dialysis, in-center hemodialysis, home dialysis, transplant, and conservative management 48 . However, this individual-level intervention did not result in an improvement in patients’ actions to pursue LDKT, and did not address barriers inherent to the larger health system 49 . The combination of the systems-level T-REX intervention that encourages communication between dialysis facilities and transplant centers and the individual-level culturally sensitive education intervention that Living ACTS provides is expected to improve access to living donor transplantation among AA patients with ESRD by affecting barriers that exist at multiple levels of the social ecology.…”

“…These observed inequities may be, at least in part, because of potentially modifiable cultural barriers. Better understanding of these barriers may allow the development of culturally competent LDKT education programs that may translate into increased LDKT rates in ethnic minorities, as previously shown [34,37,41,42,[54][55][56][57][58][59].…”

“…Similar homebased education programs and using culturally competent transplant education in dialysis units improved transplant knowledge and readiness to pursue LDKT for African Americans in the US [41,42]. In the US, targeted programs for Hispanics [88] and African Americans [54] have been successful in increasing LDKT patient knowledge. Innovative approaches such as "Live Donor Champion" [89], that provides support for the patient to communicate with donor candidates, and the "Transplant Ambassador Program" [90] that involves experiential peer support from LDKT recipients and donors, are novel and potentially effective approaches to improve LDKT education.…”

“…For example, “proximal” interventions in CKD patients early in the transplant process have had limited success in increasing LDKT rates, especially given the potentially years of time between delivery of the intervention and receipt of a LDKT. 11 , 13 , 14 Other interventions have been underpowered, targeting CKD patients at transplant centers that usually perform few LDKTs in Black people. 16 , 18 , 20 , 21 Social media apps, such as Facebook, do hold some promise.…”
