Effectiveness of modified dural incision to preserve the patency of the occipital sinus in foramen magnum decompression for a patient with Chiari malformation type I

Abstract: Background:Foramen magnum decompression (FMD) has been acknowledged as a standard surgical procedure for symptomatic patients with Chiari malformation type I (CM-I). However, even if dural incision is necessary during FMD, the procedure of cutting off the occipital sinus has not been regarded as a safe option.Case Description:A 27-year-old woman with intractable occipital headache was diagnosed with CM-I without syringomyelia. Preoperative examination revealed a large oblique occipital sinus on her right side.… Show more

“…This type of prominent OS was reported in several case reports, and the OS made surgery via suboccipital craniotomies problematic because of potentially fatal complications such as massive bleeding and brain swelling. [21][22][23][24]29,30 In the present study we encountered 7 cases of CM-I with dominant OS out of 213 cases (3.3%). In all these cases the OS could be considered an OOS.…”

“…30 Therefore, unilateral duraplasty could be the first option for the low-risk group. 29,31 In the unilateral OOS with hypoplastic ipsilateral TS type, duraplasty could be dangerous because there is no ipsilateral alternative pathway when the OOS is injured. In these cases, fourth ventricle-subarachnoid shunt or unilateral duraplasty on the contralateral side could be an option.…”

“…Various modifications of the dural incisions have been described to preserve the sinus patency in Chiari malformation [11, 13], vestibular schwannomas, and other posterior fossa tumors (medulloblastomas, ependymoma, etc.) [10, 14].…”
