“…RCTs have found that adding oestrogen, progesterone, or tibolone significantly relieves hot flushes caused by gonadotrophin releasing hormone analogues (by 50% or more on symptom scores) 13 24 25 . Danazol: Adverse effects occurred in 15% of women taking danazol 15.…”

“…The non-significant decrease of the lumbar spine BMD in our patients at the end of the GnRH-a/tibolone treatment seems to be in the same range observed after use of GnRH-a combined with other add-back regimens containing estrogens and progestins [21]; however, there are studies showing that estrogen-based add-back therapy failed to prevent bone mineral density loss after prolonged use of GnRH-agonist [22]. Concerning tibolone, two randomized, placebo-controlled trials found that tibolone 2.5 mg per day as add-back therapy seems to be effective in preserving bone density in patients treated with GnRH-agonists [23,24], whereas a histomorphometric study showed that increased bone turnover during GnRH-a treatment can be prevented by tibolone [25].…”

“…Moreover, the beneficial effects of tibolone on mood and quality of life can be explained not only with the reduction of vasomotor symptoms but also by direct action on the CNS. Different mechanisms by which tibolone acts on the CNS have been hypothesized (58)(59)(60)(61). In fact, in animal and in human models, it has been demonstrated that tibolone's metabolites act on the brain with several site-specific actions (59).…”
