PurposeTo determine the indications, long-term clinical and visual outcomes, and complications of the aphakic snap-on type I Boston keratoprosthesis (KPro).DesignRetrospective, non-comparative case series.MethodsForty-five eyes of 43 patients with type I aphakic snap-on KPros with at least 1 year of follow-up were included. The past medical histories, preoperative indications, best-corrected visual acuities (BCVAs), postoperative complications, and retention rates were analyzed.ResultsThe most common indication for KPro implantation was a failed corneal graft (89%). The mean preoperative BCVA was count fingers–hand motion (2.14±0.45 logarithm of minimum angle of resolution [logMAR]), which initially improved to 20/200 (1.04±0.85 logMAR; P<0.0001). At the last examination, 24 eyes (53%) maintained some visual gain, 22% retained their preoperative visual acuity, and 24% lost vision due to postoperative events and underlying ocular comorbidities. Postoperative complications included retroprosthetic membranes (8/45, 18%), corneal melts (5/45, 11%), glaucoma progression (6/45, 13%), and endophthalmitis or sterile vitritis (6/45, 13%). The KPro retention rate was 89%, with a mean follow-up of 51 months. The mean BCVA at the last visit was 20/1,400 (1.82±0.92 logMAR).ConclusionMost patients experienced improved visual acuity after the implantation of the aphakic, snap-on type I KPro; however, the visual gains were not sustained over time, correlating with the onset of postoperative complications.

A non-comparative retrospective case-series study was performed on 6 patients (12 eyes) with a confirmed diagnosis of ocular cicatricial pemphigoid (OCP), treated with MicroPulse Transscleral Laser Therapy (MP-TLT), between January 2018 and 2020, with a minimum follow-up of 2 years, in a private ophthalmology practice with offices in Argentina and Colombia. Mean age was 75±6.3 years (66–83 yrs). The severity of ocular cicatricial pemphigoid remained stable 2 years after MP-TLT. Baseline mean IOP was 19.5±5.9 mmHg (13–30 mmHg) and, 24 months after treatment, IOP decreased to a mean of 13.9±1.9 mmHg (10–18 mmHg). The mean IOP reduction was 25.1% (P: 0.002). The mean number of anti-glaucomatous drugs at baseline was 3.1±0.7, decreasing to 0.6±0.7, 2 years after MP-TLT (P<0.001). MP-TLT may be considered as a non-incisional approach to glaucoma care for patients with ocular surface diseases, prospective long term studies are needed.

BackgroundThe software supplied with the Corvis tonometer (which is designed to measure intraocular pressure with the use of the air-puff method) is limited to providing basic numerical data. These data relate to the values of the measured intraocular pressure and, for example, applanation amplitudes. However, on the basis of a sequence of images obtained from the Corvis tonometer, it is possible to obtain much more information which is not available in its original software. This will be presented in this paper.Material and methodThe proposed software has been tested on 1400 images from the Corvis tonometer. The number of analysed 2D images (with a resolution of 200 × 576 pixels) in a sequence is arbitrary. However, in typical cases there are 140 images. The proposed software has been written in Matlab (Version 7.11.0.584, R2010b). The methods of image analysis and processing and in particular edge detection and the fast Fourier transform have been applied.Results and discussionThe software allows for fully automatic (1) acquisition of 12 new parameters previously unavailable in the original software of the Corvis tonometer. It also enables off-line (2) manual and (3) automatic browsing of images in a sequence; 3D graph visualization of: (4) the corneal deformation and (5) eyeball response; 6) change of the colour palette; (7) filtration and (8) visualization of selected measured values on individual 2D images. In addition, the proposed software enables (9) to save the obtained results for further analysis and processing.ConclusionsThe dedicated software described in this paper enables to obtain additional new features of corneal deformations during intraocular pressure measurement. The software can be applied in the diagnosis of corneal deformation vibrations, glaucoma diagnosis, evaluation of measurement repeatability and others.
