Effects of a nonpeptide motilin receptor antagonist on proximal gastric motor function

Abstract: AimTo assess the effects of the motilin receptor antagonist RWJ-68023 on basal and motilin-stimulated proximal gastric volume. MethodsEighteen healthy male volunteers received RWJ-68023 in two different doses or placebo for 135 min. After 45 min, subjects received a motilin infusion for 90 min. Proximal gastric volume was measured with a barostat at constant pressure and during isobaric distensions. Abdominal symptoms were scored using visual analogue scales. Motilin and RWJ-68023 concentrations were assessed … Show more

“…TZP-201 reduced motilin-induced contractions of the colon in S. murinus 228 . In humans, RWJ-68023 antagonized the tonic effect of motilin on the proximal stomach 229 . None of these antagonists have been advanced to large clinical trials, at least in part because of a lack of established end points (patient-reported outcomes) and issues with patient selection and magnitude of therapeutic effect.…”

“…MA-2029, GM-109, TZP-201 and RWJ-68023 are all motilin receptor antagonists [226][227][228][229] . In dogs, MA-2029 inhibited gastrointestinal motility induced by motilin without affecting gastric emptying 230 .…”

“…It is unlikely that motilin is released in sufficient amounts during eating to significantly affect gastric motility in healthy volunteers (Boivin et al ., ). This view appears to be consistent with the observation that the motilin receptor antagonist RWJ‐68023 did not affect proximal gastric volume in healthy volunteers (Kamerling et al ., ). However, the latter experiments should be treated with caution because even high doses of RWJ‐68023 only partially prevent the ability of motilin to contract the stomach.…”

“…Motilin, a 22-amino-acid polypeptide, is related to the regulation of gastrointestinal motility (Itoh, 1997) and new chemical entities which act on motilin receptors have recently been expected for the treatment of functional gastrointestinal tract disorders (McCallum and Cynshi, 2007;Kamerling et al, 2004). Irritable bowel syndrome (IBS) is a functional bowel disorder in which abdominal discomfort or pain is associated with altered bowel habits (Thompson et al, 1999).…”
