Electroacupuncture (EA) has been shown to suppress premature ventricular complexes (PVCs) following myocardial infarction (MI) in humans. However, the specific neural circuitry and causal mechanisms underlying this effect remain unclear. Here, we reveal a previously unrecognized connection from the primary motor cortex (M1) to the nucleus rostral ventrolateral medulla (RVLM) circuitry via the layer 5 of the primary motor cortex (M1L5)-zona incerta (ZI) pathway, which selectively suppresses PVCs in post-MI mice. Utilizing viral tracing, fiber photometry recordings, and optogenetic stimulation, we demonstrate that EA inhibits glutamatergic projections from M1L5 to ZI, leading to the activation of local GABAergic neurons and subsequent inhibition of RVLM (M1L5-ZI-RVLM). Furthermore, optogenetic or chemogenetic inhibition of the M1L5-ZI-RVLM circuit replicates the anti-PVC effects observed with EA in MI mice. Artificial activation of M1L5-projecting ZI neurons reverses the suppressive effects of EA on PVCs in MI mice. Overall, our findings highlight the M1L5-ZI-RVLM circuit as a crucial mediator of EA-induced suppression of PVCs following myocardial infarction. Additionally, this newly identified corticothalamic circuit may represent a promising target for mitigating PVCs post-myocardial infarction.

BackgroundAlcohol use disorder (AUD) has become a significant global factor in various diseases. As a non-pharmacological therapy, certain therapeutic potential has been found in acupuncture; however, in-depth mechanistic studies related to acupuncture for patients with AUD are still insufficient.MethodsBased on a randomized control design and a multi-omics analysis plan, this protocol details the recruitment (42 AUD patients), group allocation (21 in acupuncture group vs. 21 in sham acupuncture group), intervention and follow-up (replacement drugs as a normal treatment, 2 weeks acupuncture duration, and 3 month follow-up), and data collection and analytical processes. For the clinical outcomes, in addition to the time required for alcohol withdrawal symptoms to subside as the primary outcome, changes in the alcohol withdrawal symptoms, alcohol craving, mood dysfunction, sleep disorder, fatigue, self-efficacy, gastrointestinal symptoms, the quality of life, and the relapse outcomes will be compared between the groups to confirm the acupuncture clinical effectiveness on alcohol withdraw. The gut microbiome and the fecal metabolomics will also be assessed to explore the association of the structure and the function of gut microflora and the mediation of acupuncture effect on AUD fully utilizing gut microflora multi-modal data and clinical information, via the combination of multi-omics methods, feature screening algorithms and appropriate models.DiscussionThe results of this study may help to strengthen clinical evidence of the mechanism of acupuncture intervention in patients with AUD, through understanding of the regulatory mechanism of acupuncture in the gut microbiome and its metabolism as well as AUD-related clinical manifestations.Trial registrationChinese Clinical Trial Registry ChiCTR2200058120. ObjectiveThe aim of this study was to observe the efficacy of electroacupuncture (EA) and psychotherapy (PT) effect on the mental status, sleep quality and impulsive trait in patients with pathological internet use, and to observe the changes of Monoamine oxidase type A (MAOA) messenger Ribonucleic acid (mRNA) levels in each group.MethodsA total of 60 PIU patients were included for the present study. These patients were randomly divided into two groups: EA group and PT group. Baihui, Sishencong, Hegu, Neiguan, Shenmen, Taichong, Sanyinjiao and Xuanzhong were selected for acupuncture in the EA group, while group psychotherapy combined with individual psychotherapy was used for intervention in patients in the PT group. Young’s Internet addiction Test (IAT), Yale-Brown Obsessive-Compulsive Scale (Y-BOCS), Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS), Barratt Impulse Scale (BIS-11) and Pittsburgh Sleep Quality Index (PSQI) were used to observe the severity of Internet addiction, mental status, sleep quality and impulsive trait of all patients at baseline and 40th days of treatment; and MAOA mRNA data were collected at baseline and 40th days of treatment.ResultsElectroacupuncture and psychological intervention effectively reduced IAT, SAS, SDS, Y-BOCS, BIS and PSQI scores of PIU patients. After 40 days treatment, the MAOA expression of the PT group was increased, and there was no significant change in EA group. The correlation analysis indicated that IAT scores were positively correlated with SAS, SDS, Y-BOCS, BIS and PSQI at baseline. In addition, after treatment the EA group showed that the change in IAT scores was positively correlated with the change in Y-BOCS and BIS scores, and the PT group showed that the change in IAT scores was positively correlated with the change in SDS, BIS and PSQI scores.ConclusionThe present study showed that electroacupuncture and psychological intervention can improve severity of Internet addiction, mental status, sleep quality and impulsive trait of PIU patients. Simultaneously, neurobiological changes may be the underlying mechanisms of psychotherapy for internet additcion.
