Effects of acute nutritional ketosis during exercise in adults with glycogen storage disease type IIIa are phenotype‐specific: An investigator‐initiated, randomized, crossover study

Abstract: Glycogen storage disease type IIIa (GSDIIIa) is an inborn error of carbohydrate metabolism caused by a debranching enzyme deficiency. A subgroup of GSDIIIa patients develops severe myopathy. The purpose of this study was to investigate whether acute nutritional ketosis (ANK) in response to ketone‐ester (KE) ingestion is effective to deliver oxidative substrate to exercising muscle in GSDIIIa patients. This was an investigator‐initiated, researcher‐blinded, randomized, crossover study in six adult GSDIIIa patie… Show more

“…The deficit in V̇O 2 peak was different amongst the group, with participants achieving between 22 and 91% of their predicted value. Hoogeveen et al [ 10 ] also identified a large range in V̇O 2 peak but with values generally higher than the current study (range: 46–105% of predicted). A key factor explaining this variation in the current study appears to be age/disease progression, with those achieving a higher V̇O 2 peak tending to be younger.…”

“…A key factor explaining this variation in the current study appears to be age/disease progression, with those achieving a higher V̇O 2 peak tending to be younger. However, in contrast, Hoogeveen et al [ 10 ] found older participants had the highest V̇O 2 peak relative to predicted. This discrepancy could be attributable to differences in the genetic and environmental characteristics of the two samples.…”

“…Following this logic, it is possible that ketone bodies may offer another therapeutic avenue. Other than the study cited above, 66 a recent trial 70 examined whether the ingestion of ketone‐esters with maltodextrin, compared to maltodextrin alone, would improve cardiorespiratory and metabolic parameters in GSD III patients during exercise. They show a notable effect in those patients with overt myopathy, specifically improvement in the respiratory exchange ratio and the 31 P MR spectra of the quadriceps during exercise at 60% of maximum capacity in the ketone‐ester group compared to the maltodextrin group.…”
