Effects of Age and Serum 25-OH-Vitamin D on Serum Parathyroid Hormone Levels

Abstract: This analysis, despite inevitable inherent limitations, introduces several clinical implications. First, median 25-OHD-dependent PTH levels revealed no threshold above which increasing 25-OHD fails to further suppress PTH. Second, the large number of subjects with 25-OHD deficiency and hyperparathyroidism reinforces the Third International Workshop on Asymptomatic Primary Hyper parathyroidism's recommendations to test for, and replete, vitamin D depletion before considering parathyroidectomy. Third, strong age… Show more

“…In agreement with the literature, an inverse logarithmic relationship between 25(OH)D and serum PTH was confirmed [36] [41]. In a survey of more than 900 Australian women [42], PTH concentration was found to be associated with ionized calcium, age, weight and 25(OH)D concentration, with the latter variable being the determinant of greater significance in the multivariate analysis (r = −0.21).…”

“…Whether the observed seasonal variation in vitamin D concentration is pathological and not normal physiology is difficult to assess. The definition of vitamin D sufficiency, often regarded as the concentration above which PTH cannot be suppressed further, varies widely (e.g., from 30 to 110 nmol/L [60]) and indeed, in the largest study to date, no threshold above which increasing vitamin D concentration failed to further suppress PTH could be identified [62]. Furthermore, the relationship between PTH and vitamin D was highly dependent on age [62].…”

Furthermore, the relationship between PTH and vitamin D was highly dependent on age [62]. The population in which PTH is most commonly measured is also known to have a high prevalence of vitamin D deficiency/insufficiency [63] and therefore use of a PTH reference range derived in vitamin D repleted individuals may be inappropriate.…”

“…The definition of vitamin D deficiency, insufficiency, and sufficiency is currently challenging as an overall consensus is still lacking (48,49,50 In recent years, a number of position statements and clinical practice guidelines have been published to define the optimal vitamin D status and the health outcomes associated with its alteration (52,53). Many different recommendations on dietary intakes needed to reach and maintain sufficient 25(OH)D levels have been proposed as well (54,55,56,57,58,59).…”

“…Actually, maximal intestinal calcium absorption, serum PTH suppression (52,53), reduced risk of falling, prevention of fractures, increase in BMD, and reduced histomorphometric findings of osteomalacia from bone biopsy are the most important parameters considered in both reports to identify the optimal vitamin D status (58,59,64,65,66). Table 1 briefly summarizes the different conclusions about these skeletal outcomes reached by the two professional organizations.…”
